For D3120 pulp caps, we have decided that we would estimate for the service as normal and have the patient sign a “BILLABLE” consent form so the patient is responsible for the procedure if the insurance company doesn’t pay according to our estimate or if they consider the service inclusive and want us to write it off. 

Every patient getting a pulp cap should sign the specific billable consent form for pulp caps at the time of service.
